Three patterns, plainly

Is it PMDD, PMS or something else?

Distress before periods deserves attention, and the first job is to see which pattern it follows.

A cycle-linked pattern

In PMDD, marked mood changes arrive in the days before a period, ease within a few days of it starting, and disrupt work, home or relationships.

A difficulty that worsens before periods

An ongoing condition such as depression or anxiety can grow heavier before a period, and then the condition itself needs care.

Another explanation

Poor sleep, a physical health problem, a medicine or a hard stretch of life can look similar, so each of these is checked too.

PMDD or PMSworse before periodsthe whole monthother causes

The distress is real, whichever name fits.

The month, in your words

What do people bring to a first conversation?

Many arrive with a pattern that keeps coming back rather than a label, and these patterns come up often.

Mood swings before periods

  • when the change starts and when it lifts
  • what the easier days look like
  • how work or study is affected

Crying, anger and rage before periods

  • what sets it off, and what helps it pass
  • who is around when it happens
  • how it feels afterwards

Sleep, energy and focus

  • sleep in the second half of the month
  • energy and concentration at work or home
  • what changes once the period starts

Who this is for

Who is this care for?

Someone whose month follows a pattern

For anyone who notices mood, anger or tears returning before a period and easing after it starts. A label is not needed to ask.

Someone already in care

For people whose depression, anxiety, bipolar pattern or ADHD feels heavier before periods. Current treatment stays in view, and medicines change only with the treating clinician.

Someone seeing a gynaecologist

Many people start with a gynaecologist, and that care continues. With your consent, we coordinate with your gynaecologist so both parts fit together.

Psychiatrist or psychologist

How it works

How does PMDD assessment work at Weave?

Four steps cover most of what happens, and the pace stays calm.

Daily notes

Brief daily notes on mood, sleep, how the day went and period dates across two cycles, easier days included. Help does not wait for the notes.

The focused assessment

A 30-minute conversation about the whole month, your health, current medicines, other conditions and safety. It ends with a shared view of the pattern.

A plan together

Options are weighed for their benefits, risks and fit with your life. Nothing is decided before your questions are answered.

Review

Agree when to review, what to watch over the next one or two cycles, and how to raise a concern between appointments.

Is there a single test for PMDD?

No single test settles the question, so timing and the conversation come first.

Hormone tests

A hormone result alone does not show PMDD, and a normal result does not dismiss the distress.

Tests for other causes

Blood tests are suggested only when the history points to another explanation, such as a physical health condition.

Online quizzes

A quiz score cannot settle the question; a whole-month view and a conversation can.

Daily notes

Notes kept day by day show timing more clearly than memory does.

Worth a second look

What else can get worse before a period?

Some conditions grow heavier in the days before a period and need care in their own right.

depressionanxietybipolar patternsPMDD and ADHDsleep problems

When an ongoing condition deepens

Depression, anxiety or a bipolar pattern that deepens before a period is reviewed in its own right. The depression, anxiety and bipolar guides explain each one.

When ADHD or sleep slips

Some people with ADHD find focus and mood harder to manage before a period, and sleep can slip in the same days. Both are worth raising, and both are assessed alongside the cycle pattern.

Naming the pattern well guides care.

What travels with it

What else can the days before a period touch?

Appetite, closeness and conflict can all change before a period, and each deserves its own conversation.

Binge eating before periods

Appetite can change before a period. Repeated loss of control around food, or guilt after eating, deserves its own assessment.

Shutting down before periods

Going quiet or pulling away can come from overwhelm, poor sleep or stress. Withdrawal that lasts beyond the period gets a closer look.

Strain at home

Arguments can cluster in the same days each month. The pattern can be talked about without blame, and a cycle pattern never excuses harm.

Plain facts for hard weeks

Arguments

Agree a pause and a time to come back to the talk, planned on a calm day. Each person stays accountable for what they say and do.

Food

Regular meals tend to help more than skipping or restricting. Shame makes eating harder, not easier.

Partners

A partner can ask what support would help, rather than guessing or keeping watch on the calendar. The partner guide has more.

Safety at home

Threats, intimidation or violence need help now, whatever the time of the month.

Treatment and follow-up

What happens after the assessment?

The plan follows what the assessment shows and what matters to you, and it can draw on more than one approach.

Therapy, including CBT

CBT has evidence for easing distress and building ways to cope in PMDD.

Therapy is available at Weave, and the approach is agreed at assessment.

SSRI antidepressants

SSRI antidepressants have evidence of benefit in PMDD from many trials.

How they are taken, and their side effects, are decided one to one and reviewed.

Hormonal and specialist options

Selected combined hormonal methods can help some people, chosen with your gynaecologist.

Ovulation suppression and, rarely, surgery are specialist steps for severe, lasting difficulty, with monitoring that includes bone health.

Thoughts of self-harm, or of not wanting to be here, can come with the days before a period, and they can be talked about openly with a clinician.

Crisis lines are listed in the crisis support section of this page. Weave is not an emergency service. We reply within one working day.

The plan follows what the months show.

Evidence, plainly

What can the evidence tell us, and what can it not?

Options differ in evidence and risk, and a plan should say why each one is offered.

What about antihistamines?

Antihistamines are not an established PMDD treatment: IAPMD's May 2026 statement found no randomised trials, judged a short over-the-counter trial unlikely to harm most who try one, and advised asking a pharmacist before combining them with other medicines.

How firm is the main evidence?

SSRI antidepressants have evidence from many trials. Selected combined hormonal pills have fewer trials, with a small to moderate benefit against placebo, low certainty and more side effects. The two have not been compared well enough to rank one above the other.

What about supplements and herbal products?

Evidence for supplements and herbal products is limited or mixed, and a natural label does not make a product safe, so it helps to discuss them first.

Common questions

Questions people ask before booking

How is PMDD different from PMS?

PMS is common and covers milder changes before a period. PMDD is less common and more severe, with low mood, anger or anxiety that disrupts work, home or relationships, then eases within a few days of the period starting.

Is mood getting worse before periods always PMDD?

No. Depression, anxiety, bipolar patterns and ADHD can all get heavier before a period without being PMDD; clinicians call this premenstrual exacerbation. Notes across the whole month help show which pattern fits, and each needs different care.

Do I need to see a psychiatrist for PMDD, or a gynaecologist?

Either can be a first stop. A psychiatrist helps most when mood, safety or another mental health condition is involved, and a gynaecologist when periods, contraception or hormonal options are central. With your consent, we coordinate with your gynaecologist.

What happens in the focused assessment?

It is a 30-minute conversation about when changes start and ease, sleep, physical health, current medicines, other conditions and safety. It ends with a shared view of the pattern and the options worth discussing next.

What should I note before I come?

A line a day is enough, covering mood, sleep, how the day went and period days. Note the easier days as well. Two cycles help, but come sooner if things feel heavy, and bring whatever notes exist.

Does stopping ovulation help PMDD?

It can help some people when other options have not, because PMDD is linked to sensitivity to normal monthly hormone changes. These specialist treatments carry risks and need monitoring, and lighter or absent bleeding does not mean ovulation has stopped.

Do antihistamines help PMDD?

Some people report feeling better on them, but no randomised trial has tested them yet, so they are not an established treatment. A lack of trials is not proof they fail. Check with a pharmacist before mixing them with other medicines.

What does PMDD treatment cost at Weave?

The Focused assessment is 30 minutes, at INR 3,000. A 20-minute session is INR 2,200 and a 30-minute session is INR 3,000, and you choose the length when you book.

What if I feel unsafe before my period?

Reach out now, as a pattern does not need to be confirmed first. Call 112 or reach the nearest emergency department if life is at risk right now. Tele-MANAS on 14416 gives mental health support by phone.
